To ensure a smooth registration process and maximize the time you can spend with your doctor, please complete the following steps before your first visit. We ask that you check in 20 minutes prior to your appointment to complete the registration process. If you have not completed the following paperwork prior to your visit, you will be asked to complete it before seeing the doctor.

Step 1: Review Checklist and Print and Complete All Forms

Please review and reference our New Patient Appointment Checklist, then print and complete all of the following forms. You will be expected to bring them to your first appointment.

• For each female patient/partner:

- Female Patient Registration Forms (print and complete)
- State of NJ Patient Rights (read and print for your records)

• For each male patient/partner:

- Male Patient Registration Forms (print and complete)
- State of NJ Patient Rights (read and print for your records)

Step 2: Bring Pertinent Medical Records

Please obtain and bring any medical records that may be pertinent to your visit. This may save you considerable time during the evaluation process. Examples of records that may be useful include but are not limited to:

• Prenatal bloodwork (blood type, genetic testing, infectious disease testing, etc.)
• Recent Pap smear and/or cervical culture results
• Records from any previous infertility evaluation or treatment performed by an outside doctor/office
• Results/films/CD from any pertinent diagnostic procedures (hysterosalpingogram, MRI, ultrasounds)
